What is Breast Augmentation?

Breast augmentation typically refers to the surgical placement of saline or silicone implants behind the breasts in order to create fuller, shapelier breasts. However, fat transfer procedures are now available, allowing women to enhance their chests naturally, using tissue harvested from other areas of the body.

Some women turn to breast augmentation surgery because pregnancy, weight loss, or aging has resulted in a loss of breast volume, while other women choose the procedure simply because they want to enhance their bust and look their best.

 Breast Augmentation with Implants

Prior to your breast augmentation, Dr. Mountcastle will determine whether saline or silicone implants are best suited for your needs and desires. Both options consist of an outer silicone shell; depending on the implant type, the shell will be filled with either saline fluid or silicone gel.

Breast augmentation is most often performed under general anesthesia, which allows patients to remain asleep and pain-free throughout the procedure. The length of the procedure is typically one hour, although surgical time will vary by case.

Incisions may be made in one of several discreet locations, including the underarm, around the areola, or at the base of the breast, known as the inframammary crease, and any resulting scars will be effectively concealed beneath underwear or bathing suit lines. The advantage to the incision under the breast is that if it scars poorly which all scars will do 5% of the time it remains hidden. If the other scars scar badly they will be more easily visible from a standing position. In general however all scars in breast augmentation are very minimal.  After the incision has been made, Dr. Mountcastle will place the implant either above or behind the pectoral muscle.  If you have chosen saline breast implants, they will be empty at the time of insertion, and filled to the desired volume after they are placed.

The incision site will then be closed with sutures and surgical tape. Dr. Mountcastle does not place drainage tubes as he does not feel they provide any benefit and may theoretically increase chance of infection or discomfort.  Antibiotics are routinely given around the time of surgery to prevent post op infections.

After Surgery

After your breast augmentation, you may experience swelling and soreness for several weeks. Wearing a supportive bra or compression bandage may help alleviate post-surgical discomfort, and Dr. Mountcastle will prescribe medication as necessary.

Patients may resume non-strenuous activities within one week, but exercise should be avoided for approximately two to three weeks. Dr. Mountcastle will give you detailed instructions to guide your post-operative care. Breast Augmentation through Fat Transfer

Breast augmentation through fat transfer consists of two procedures performed on the same day. The first procedure harvests the fat from the donor site, removing necessary tissue and reshaping the waist, thighs, or other chosen area for more attractive contours. The second procedure implants the harvested fat into the breasts through several small micro puncture-sites. Dr. Mountcastle is proud to be one of a handful of surgeons with the knowledge of fat-grafting techniques and eye for aesthetic enhancement necessary to perform this exciting procedure.

Having attended the international fat grafting congress and training with one of the pioneers of fat grafting Dr. Kamran Khoobehi, Dr. Mountcastle is uniquely positioned to offer such a new technique. Since 1995 when Dr. Sydney Coleman of New York performed the first modern day breast enhancement with autogenous fat the promises of such a procedure have been on the rise.  For a number of years, many plastic surgeons remained skeptical about these techniques interfering with mammographic surveillance of breast and whether the procedure worked or not.  These have been shown to be not true with many recent papers.
